MedDeskOS — Doctor-owned clinical OS

Feature · OPD queue

A queue your patients see. A console your doctor controls.

Live token queue with call-next, engage-timestamp, walk-in registration, and skip/no-show — driven by Server-Sent Events so reception and doctor stay perfectly in sync.

The problem we solved

Most clinic queues run on a whiteboard or, worse, the receptionist's memory. Doctors call into the front desk asking who's next. Patients in the waiting room have no idea where they stand. Token numbers disagree across staff.

We modelled it as a state machine: every appointment moves Scheduled → Checked-in → In consult → Billed → Closed, and every staff role sees the same authoritative view.

What's in the box

Today's queue, live

Tokens grouped by status (Waiting / In progress / Done / Skipped), sorted by token number. Updates within milliseconds across every browser open on the same clinic — Server-Sent Events, not polling.

Call-next, one button

Doctors hit Call Next; the system marks the current patient Done, promotes the next Waiting token to In Progress, and stamps an engagedAt timestamp. Pickup delay (called → actually engaged) becomes a measurable clinic-quality metric.

Walk-in flow

Reception creates an appointment + queue token in one form: pick the doctor, optional chief complaint, done. Walks straight into the Visit Console for the doctor to engage.

Skip + no-show

Skip moves a token to the bottom of the day's list (e.g. patient stepped out). No-show closes the slot definitively. Both write an AuditLog row.

Estimated wait time

Each Waiting token shows ~10 minutes per patient ahead — adjustable per clinic. Manageable expectation-setting beats a vague "please wait."

Why this matters for an Indian clinic

Front-desk staff burn 20–30% of their day answering "how long is the wait?" A live token display in the waiting room (any tablet pointed at the queue screen) absorbs most of that question.

And when a doctor is running an hour late, the receptionist isn't taking the heat — the queue is already showing it. Patients self-rebook. Conflict avoided.
